The "Heart&Stroke Healthy Living Calendar" sweepstakes (the "Sweepstakes") is sponsored by the Heart and Stroke Foundation of Canada (the "Sponsor"). The Sponsor thanks the participating companies for their support. Pacific Cycle Inc., a division of Dorel Industries Inc, is the sole supplier of all prizing and the Sponsor accepts no liability associated with the Sweepstakes, prizing or related items. The Sweepstakes begins at 12:00:01 a.m. (ET) on November 01, 2008 and ends at 11:59:59 p.m. (ET) on December 31, 2009 (the "Sweepstakes Period").

1. TO ENTER: No Purchase Necessary. Residents of Canada who are the age of majority may enter the Sweepstakes. Log onto healthcheck.org select the "Health Check" Sweepstakes icon and complete the online entry form with full name, complete mailing address (including postal code), daytime telephone number (including area code) and a valid email address. There will be a monthly sweepstakes (the "Monthly Sweepstakes") for each of the monthly sweepstakes periods set out in section 3 (collectively, the "Monthly Sweepstakes Periods" and individually, the "Monthly Sweepstakes Period").

   i. to enter the Monthly Sweepstakes for each of the Monthly Sweepstakes Period of November 2008, December 2008, January 2009, February 2009, March 2009, May 2009, June 2009, October 2009 and December 2009, enter the UPC code found on a participating product for the applicable Monthly Sweepstakes Period (for a list of participating products, visit healthcheck.org and select the Health Check Sweepstakes icon for a list of participating products). Products do not need to be purchased to obtain UPC codes.

   ii.to enter the Monthly Sweepstakes for each of the Monthly Sweepstakes Period of April 2009, July 2009, August 2009, September 2009 and November 2009, include the Recipe Title found on the Heart and Stroke Healthy Living Calendar 2009 (the "Calendar") for the applicable Monthly Sweepstakes Period (see Section 3).

   To view the Calendar visit healthcheck.org and click on the Health Check Sweepstakes icon. Upon doing so, confirm that you have read the official rules and regulations of the "Heart&Stroke Healthy Living Calendar" Sweepstakes (the "Official Sweepstakes Rules and Regulations") and click the "submit" icon to be automatically entered into the Sweepstakes. All entries must be received on or before 11:59 PM (ET) on the relevant cut-off-date as described in Section 3.

   Limit of one (1) entry per person per Monthly Sweepstakes Period.

2. PRIZES: There will be a total of fourteen (14) monthly prizes available to be won (one (1) each month during the Sweepstakes Period), each consisting of one (1) Schwinn Bicycle as selected by Pacific Cycle Inc., a division of Dorel Industries Inc. (collectively, the "Monthly Prizes" and individually, the "Monthly Prize"). The approximate retail value of each Monthly Prize is $500.00 (CDN). Monthly Prizes must be accepted as awarded without substitution and are not transferable, not for resale and have no cash surrender value. The Sponsor reserves the right, in the event that a Monthly Prize or any component of a Monthly Prize cannot be awarded as described for any reason, to substitute another prize or component of a prize of equal or greater value, without liability. Limit of one (1) Monthly Prize per person.
3. MONTHLY SWEEPSTAKES PERIODS (each a Monthly Sweepstakes Period):
   Month 1: begins 12:00:01 a.m. (ET) November 01, 2008 and ends 11:59:59 p.m. (ET) November 30, 2008;
   Month 2: begins 12:00:01 a.m. (ET) December 01, 2008 and ends 11:59:59 p.m. (ET) December 31, 2008;
   Month 3: begins 12:00:01 a.m. (ET) January 01, 2009 and ends 11:59:59 p.m. (ET) January 31, 2009;
   Month 4: begins 12:00:01 a.m. (ET) February 01, 2009 and ends 11:59:59 p.m. (ET) February 28, 2009;
   Month 5: begins 12:00:01 a.m. (ET) March 01, 2009 and ends 11:59:59 p.m. (ET) March 31, 2009;
   Month 6: begins 12:00:01 a.m. (ET) April 01, 2009 and ends 11:59:59 p.m. (ET) April 30, 2009;
   Month 7: begins 12:00:01 a.m. (ET) May 01, 2009 and ends 11:59:59 p.m. (ET) May 31, 2009;
   Month 8: begins 12:00:01 a.m. (ET) June 01, 2009 and ends 11:59:59 p.m. (ET) June 30, 2009;
   Month 9: begins 12:00:01 a.m. (ET) July 01, 2009 and ends 11:59:59 p.m. (ET) July 31, 2009;
   Month 10: begins 12:00:01 a.m. (ET) August 01, 2009 and ends 11:59:59 p.m. (ET) August 31, 2009;
   Month 11: begins 12:00:01 a.m. (ET) September 01, 2009 and ends 11:59:59 p.m. (ET) September 30, 2009;
   Month 12: begins 12:00:01 a.m. (ET) October 01, 2009 and ends 11:59:59 p.m. (ET) October 31, 2009;
   Month 13: begins 12:00:01 a.m. (ET) November 01, 2009 and ends 11:59:59 p.m. (ET) November 30, 2009; and
   Month 14: begins 12:00:01 a.m. (ET) December 01, 2009 and ends 11:59:59 p.m. (ET) December 31, 2009 (the date on which each Monthly Sweepstakes Period ends is the "Monthly Sweepstakes Closing Date").
4. MONTHLY PRIZE DRAWS: Each month during the Sweepstakes Period, one (1) selected entrant for the preceding Monthly Sweepstakes Period will be chosen at random from the list of eligible entrants received. Monthly prize draws will be held at 2261 Royal Windsor Drive, Unit C, Mississauga, ON at approximately 10:00 a.m. (ET) on the following dates: December 08, 2008 for Month 1; January 08, 2009 for Month 2; February 10, 2009 for Month 3; March 10, 2009 for Month 4; April 08, 2009 for Month 5; May 07, 2009 for Month 6; June 10, 2009 for Month 7; July 08, 2009 for Month 8; August 11, 2009 for Month 9; September 08, 2009 for Month 10; October 08, 2009 for Month 11; November 10, 2009 for Month 12; December 08, 2009 for Month 13 and January 12, 2010 for Month 14 (collectively, the "Monthly Prize Draws" and individually, the "Monthly Prize Draw"). Each Monthly Prize Draw will take place from among all eligible entries received during the applicable Monthly Sweepstakes Period (see Section 3). Odds of being selected for a Monthly Prize depend on the total number of eligible entries received during the applicable Monthly Sweepstakes Period. PLEASE NOTE: Entries will not be carried over into the following month’s draw. Each selected entrant will be contacted by mail or telephone within ten (10) business days of the applicable monthly prize draw date (the "Monthly Prize Draw Date"). If a selected entrant cannot be reached within ten (10) business days of the applicable Monthly Prize Draw Date, the selected entrant will be disqualified and another entrant will be selected until such time as contact is made by mail or telephone with a selected entrant or there are no more eligible entries, whichever comes first. The Sponsor will not be responsible for failed attempts to contact a selected entrant.
5. ELIGIBILITY: To enter and be eligible to win, a person must be a resident of Canada who has reached the age of majority in the jurisdiction in which he/she resides and who is not an employee of the Sponsor and/or the Heart and Stroke Provincial Foundations, their affiliated companies, agents or representatives, their respective advertising or promotional agencies, the independent contest organization or a member of any such employee’s immediate family (regardless of where they live) or persons with whom they are domiciled (whether related to the employee or not). In the Official Sweepstakes Rules and Regulations, "immediate family" includes the following individuals: mother, father, brothers, sisters, sons, daughters, partner or spouse.
6. GENERAL: By entering the Sweepstakes, entrants agree to abide by the Official Rules and Regulations and the decisions of the independent contest organization, with respect to all aspects of the Sweepstakes, which are final.
7. To be declared a Monthly Prize Winner, a selected entrant must: (i) answer correctly, without assistance of any kind, whether mechanical or otherwise, a mathematical skill testing question; and (ii) sign a declaration and release form (the "Release Form") confirming compliance with the Official Sweepstakes Rules and Regulations, confirming acceptance of the Monthly Prize as awarded, without substitution, and releasing the Sponsor, the Heart and Stroke Provincial Foundations, its affiliated and related companies, advertising and promotional agencies, the independent contest organization and their respective shareholders, directors, officers, employees, agents, representatives, successors and assigns (collectively, the "Discharged Parties") from any liability in connection with a Monthly Prize or the Sweepstakes. The Release Form must be returned within the time period specified on the Release Form or the Monthly Prize will be forfeited. Pacific Cycle Inc., a division of Dorel Industries Inc, will contact the confirmed winners within thirty (30) days of being declared a Monthly Prize Winner to coordinate prize fulfillment.
8. None of the Sponsor or the Heart and Stroke Provincial Foundations accepts any responsibility for loss, damage or claims caused by or resulting from the Sweepstakes or a Monthly Prize. By entering the Sweepstakes and accepting a Monthly Prize, each winner consents to the use of his/her name and photo, in connection with any publicity carried out by or on behalf of the Sponsors with respect to the Sweepstakes.
9. The inclusion of a participating company in the Calendar does not represent an endorsement of the products by the Sponsor.
10. The Sponsor and the independent contest organization will not be responsible for late, lost, illegible, incomplete or destroyed entries; all such entries are void. The Sponsor and the independent contest organization do not assume any responsibility for incorrect or inaccurate capture of entry information, technical malfunctions, human or technical error, printing errors, lost, delayed or garbled data or transmissions, omission, interruption, deletion, defect or failures of any telephone or computer lines or networks, computer equipment, software or any combination thereof. Entry material/data that have been tampered with or altered are void. In the event it is determined that an entrant has entered in a fashion not sanctioned by the Official Sweepstakes Rules and Regulations and/or has submitted more than the number of entries permitted by the Official Sweepstakes Rules and Regulations, the entrant will be disqualified and all of the entries submitted by the entrant will be disqualified.
11. The Sponsor reserves the right, with the consent of the Régie des alcools, des courses et des jeux (the "Régie") in the province of Québec, to terminate or amend the Sweepstakes at any time and in any way, without prior notice. Without limiting the foregoing, if, for any reason, the Sweepstakes is not capable of running as originally planned, due to tampering for example, the Sponsor reserves the right, with the consent of the Régie, to cancel the Sweepstakes and conduct a random draw from all previously received eligible entries received by the Monthly Sweepstakes Closing Date.
12. For Québec residents only: Any litigation respecting the conduct or organization of a publicity contest may be submitted to the Régie des alcools, des courses et des jeux for a ruling. Any litigation respecting the awarding of a prize may be submitted to the Régie only for the purpose of helping the parties reach a settlement.
13. All entries become the permanent property of the Sponsor and none will be returned. No correspondence will be entered into, except with the selected entrants. The Sweepstakes is subject to all applicable federal, provincial and municipal laws.
14. The Sponsor respects your right to privacy. Personal information collected from entrants will only be used by the Sponsor to administer the Sweepstakes and, only if consent is given at the time of entry, to provide the entrants with information regarding upcoming promotions and/or events from the Sponsor. If entrants provide consent when entering the Sweepstakes, entrants consent to the manner of collection, use and disclosure of personal information as set out in the Sponsor privacy policy, which is available at www.heartandstroke.ca
